For those of you who have received IUD payment assistance from the Arch Foundation, what are the income requirements?? Thanks!

i received a mirena iud through the arch foundation, but i cant remember the eligibility requiremetns. i believe you have to be able to prove your insurance wont cover it or that you are uninsured. i know that when i had questions and called them they answered the phone and were very helpful.
