Cut it gradually - I usually get mine and cut whittle away at them until they're a comfortable fit. If it's too big, you can always cut it smaller, but you can't make it bigger once it's too small! :)
Try to maintain a sort of roundish, oval shape, because that's easier to use - but other than that, the size depends on what feels comfortable to you.

i generally don't cut them - the bigger they are the more they absorb. try inserting it during your next period and see how it feels. if it's uncomfortable, then trim a small amount, as mangofandango said. but if not i'd recommend just leaving it as is.
