Well I don't know much about YI.. but that looks pretty bad. I would see about making an appointment with the dr. I don't know if you need to go to the gyno.. a GP (your family dr) or maybe even a clinic might be able to give you a script for it.
I do know that you shouldn't be picking at it.. I would leave my hands away from it as much as possible. That way you aren't risking giving yourself an even worse infection (by maybe breaking the skin and bacteria from your hands/bowel movements/urine getting in there).
