the urethra, vagina and anus are in a line from front to back, in that order. I've never had a tampon refuse to come out unless I was not on my period (usually they are so full of blood and stuff that they slide out pretty easily). I don't know exactly why you had so much pain and trouble. Perhaps using a little lube (like KY) would help?
