First of all, it's totally normal that the pessary came out. What goes up must come down. The medicine doesn't really "go" anywhere, it comes out eventually. It's messy and gross, but normal.
And one of the side effects of Clotrimazole is "irritation" (http://www.drugs.com/sfx/clotrimazole-side-effects.html), which very well could lead to slight bleeding. But it also is possible that it is breakthrough bleeding, as archangelbeth mentioned.
