I actually once heard about the mother of a friend of mine, who works as a nurse for mentally handicapped people. At sompe point, a woman was brought to them complaining about vaginal pains and smelling really weird down there; turned out she'd had a tampon in for almost a month but forgot all about it. No toxic shock though, but I suppose this woman actually got lucky.
