Though I don't have the answers to your first few questions, to my knowledge Implanon is not yet approved in Canada. It is available in Australia and in a number of EU countries; the United States approved it last summer and it is expected to be available in the States by 2007.
I may be wrong on that, but I'm pretty sure about it. However, if the US FDA approved Implanon that generally means that a Canadian approval will follow at some point; currently, a search for "Implanon" on Health Canada's site turns up no results.
