That sounds like what happened when I had an ovarian cyst burst. I don't have endometriosis, so they were doing various scans and tests for appendicitis, kidney stones, cysts, torsion, etc... they ended up not having a conclusive diagnosis but the nurse told me it was consistent with cysts bursting, but they couldn't really diagnose since burst cysts didn't tend to leave any evidence. Wheefun.
The key for me with going in to the ER was when Vicodin -- which I wasn't taking regularly at that point -- did nothing, even when I took a double dose. I had no tolerance for it then and it was basically my "ohshit, this is serious" call, since the pain was localized and in the area that appendicitis was a concern.
